dc.description.abstractLa Artritis Reumatoide (AR) es una enfermedad autoinmune sistémica caracterizada por un estado de inflamación crónica y deterioro progresivo de las articulaciones. Su diagnóstico se basa en una serie de criterios clínicos y pruebas de laboratorio. Este último ítem abarca la detección del Factor Reumatoide (FR) y/o los autoanticuerpos anti-proteínas citrulinadas (anti-CCP), asociados a la actividad de la enfermedad y al pronóstico de la misma. Sin embargo, existen grupos de pacientes con elevado daño articular y niveles indetectables de FR y/o autoautoanticuerpos anti-CCP, dificultando el diagnóstico y las opciones terapéuticas. De forma reciente se descubrieron los autoanticuerpos anti-proteínas carbamiladas (anti-CarP) asociados a esta enfermedad, los cuales permitirían tipificar la AR en estos grupos de pacientes e individuos con condiciones de riesgo, además de poder predecir el desenlace articular. De esta manera se facilitaría el diagnóstico, así como en el tratamiento y el pronóstico. En este estudio se midieron los niveles de autoautoanticuerpos anti-CCP. Así mismo, se midieron los niveles de autoanticuerpos frente al péptido carbamilado de fibrinógeno (empleando el péptido nativo como control), en pacientes con artritis reumatoide temprana y familiares en primer grado de consanguinidad, para establecer su asociación frente al daño reumatológico y periodontal. Los resultados resaltan la asociación entre el número de articulaciones dolorosas e inflamadas y el daño periodontal con la presencia de los anticuerpos anti-péptido de fibrinógeno carbamilado en el grupo de familiares en primer grado de consanguinidad. No hubo asociación entre estos autoanticuerpos y los autoautoanticuerpos anti-CCP en los pacientes con artritis reumatoide temprana. Como hallazgo adicional, se observó que los dos grupos de estudio presentan autoanticuerpos contra el péptido nativo de fibrinógeno. En conclusión, la presencia de autoanticuerpos contra péptidos/proteínas carbamiladas están presentes en la población colombiana con artritis reumatoide temprana y familiares en primer grado de consanguinidad; asociados a manifestaciones de daño articular y periodontal. Además, la presencia de autoanticuerpos frente al péptido de fibrinógeno nativo puede ser un indicio de desarrollo de la enfermedad.spa
