Diseño del programa de seguridad del paciente para las unidades transfusionales colombianas independientes al SOGCS

Abstract

El presente proyecto proporciona a las unidades transfusionales independientes del SOGC una base documental para hacer posible la implementación de un programa de seguridad del paciente acorde a las políticas y lineamientos dictados en la materia por el Ministerio de Salud en Colombia. Objetivo: Diseñar el Programa de Seguridad del Paciente para las Unidades Transfusionales Colombianas Independientes al SOGCS. Metodología: Desde un enfoque cualitativo se da un alcance descriptivo al proyecto con un diseño de tipo no experimental y de corte transversal. Se utilizó la recolección de datos nacionales y globales con los que de manera robusta y pertinente se pudiera estructurar el pretendido programa. Resultados: Se documentaron, acorde a una revisión bibliográfica, conceptos, lineamientos, y definiciones que apoyaran la conformación del programa y mediante la construcción de una matriz de formulación, fue definida la política institucional de seguridad del paciente como parte fundamental de la plataforma estratégica de la Unidad transfusional. Por otro lado, fueron definidos para el programa en mención los siguientes elementos estructurales: Política institucional, sistema de reporte de eventos adversos y aprendizaje, cultura de seguridad, análisis y gestión de incidentes adversos y gestión del riesgo. Finalmente, se dio origen al documento propiamente descrito como Programa de seguridad del Paciente, acorde al entorno y funcionamiento de las Unidades Transfusionales en Colombia. Conclusiones: Sin lugar a duda, el compromiso con la seguridad del paciente debe ser manifestado y gestionado por todos los actores del sistema ya que a pesar de que algunos no tengan contacto directo con el paciente hacen parte de una cadena completa y articulada de suministro que es susceptible de fallas las cuales pueden extrapolarse y llegar a impactar la salud e integridad del paciente.

The present project provides the independent transfusion units of the SOGC with a documentary basis to make possible the implementation of a patient safety program in accordance with the policies and guidelines dictated in the matter by the Ministry of Health in Colombia. Objective: Design the Patient Safety Program for Colombian Transfusion Units Independent to the SOGCS. Methodology: From a qualitative approach, a descriptive scope is given to the project with a non-experimental and cross-sectional design. The collection of national and global data was used with which the intended program could be structured in a robust and relevant way. Results: Concepts, guidelines, and definitions that support the conformation of the program were documented according to a bibliographic review and through the construction of a formulation matrix, the institutional policy of patient safety was defined as a fundamental part of the strategic platform of the transfusion unit. On the other hand, the following structural elements were defined for the program in intention: Institutional policy, reporting system of adverse events and learning, safety culture, analysis and management of adverse incidents and risk management. Finally, the document properly described as the Patient Safety Program was created, according to the environment and operation of the Transfusion Units in Colombia. Conclusions: Without a doubt, the commitment to patient safety must be manifested and managed by all the actors of the system since although some do not have direct contact with the patient they are part of a complete and articulated supply chain that it is susceptible to failures which can be extrapolated and can impact the patient's health and integrity.
